The sensitive and thin skin around the eyes is the first area affected by time and is where its signs are most visible. However, the eyes are the center of one’s personality and their appearance has a significant effect on a person’s looks.

Although the first wrinkles around eyes appear after a certain age, extrinsic factors (environment, exposure to solar radiation) as well as heredity have a major role in premature aging of the eyelids, the formation of black circles or the so-called “bags” under the eyes.

Aging in upper and low eyelid is due to a combination of loose skin and fat protrusion, while this causes the appearance of eye strain and bags. Aging on the upper eyelids creates a “hood” that covers the eye. In the lower eyelid, the most common complain has to do with is the swelling of the eyes or the “bags”. All these eye problems can be corrected invasively on the eyelid, by removing excess skin or fat (blepharoplasty)

This results in laxity of the eye muscle and quite often affects the shape of the eyes (from almond-shaped to round eyes) or even vision when upper eyelid ptosis affects peripheral vision. For this reason we advise our patients, for excellent results, not to hesitate to proceed with blepharoplasty with the first visible changes.
However, both stress and cram, play a major role, making us look distressed and our gaze less alive and more tired.

In ABH Medical Group, blepharoplasty is performed by the specialised plastic surgeon Dr. Athanasiou who emphasizes on giving the eyes a more relaxed, younger look, positively contributing to the improvement of the overall appearance and psychology of the patient.

Blepharoplasty is often performed in combination with other procedures (such as a facelift or a brow lift). In this case need general anesthesia is needed. However, if it is restricted to the eye area, it can be done with local anesthesia. During surgery, which lasts about 30 minutes, the “excess” dermal and fatty tissue is removed from both the top and the bottom of the eye if it is necessary.

At the end of the surgery, small sutures are placed and removed after a week. Blepharoplasty is not considered as a painful surgery. The patient may have a feeling of weight that subsides after the first few days.

In the upper eyelids, the incision is being applied on the natural line of plication, which is located about 8-10 mm above the lashes, in order to correct both excess skin and extra fat. In the lower eyelids the technique is done in two ways. If the problem is the bags, without loose skin, transconjuctival lower blepharoplasty can be applied.

During this procedure the incision is made inside the eyelid, the specialized surgeon removes the excess fat, which is responsible for the bags and leaves no visible scar.

Alternatively when there is loose skin, beyond bags, the incision is made just below the lashes, where it is very well hidden, so that it is not visible.

Eyelid Surgery Instructions

1. Photos of the area are taken.

2. Twelve (8) hours prior to the procedure, stop eating.

3. Six (3) hours prior to surgery, no fluids are to be taken 

4. No medication shall be taken on the morning of the operation until the anaesthetist has agreed.

5. Alcohol is discontinued one (1) week prior to surgery.

6. In the event of intoxication or general anesthesia, a meeting with the anesthesiologist is required prior to surgery. 

7. No aspirin, anti-inflammatory medications or herbal supplements are taken ten (10) days prior to surgery, as all of these increase the possibility of bleeding during and after the procedure.

8. Three (3) days prior to surgery and every day the patient washes the face with medicated soap.

Postoperative picture

Although the periocular area is a very sensitive area, the surgery does not hurt during or after the surgery. Excess fat around the eyes is removed and the flow of the natural lines is not disturbed. The only thing one notices is a feeling of pressure or mild discomfort since the area immediately postoperatively swells. After the procedure, the patient may be slightly swollen in the face for a week. After blepharoplasty the expression remains the same. 

The classic postoperative picture includes swelling and bruising (bruising) in the area of the eyelids, cheekbones and cheekbone. The swelling and bruising is most pronounced in the first two to three days, and towards the end of the week the bluish colour around the eyes slowly fades and is replaced by a pale yellow colour (due to the absorption of microhaematomas). Sometimes after the operation there may be blurred vision, which may be accompanied by tearing or mild sensitivity to light. These symptoms disappear automatically within a few days.

Instructions after the operation

After the procedure, the patient is advised to use ice packs in the eyelid area to reduce bruising and swelling. The patient should spend the first two days resting as much as possible, relaxing on the couch with the head in a high position and ice cubes in the eyes. The ecchymoses begin to subside by the third 24 hours.  Someone may, walk around immediately wearing sunglasses. For the first 3 to 4 weeks, the patient should avoid activities that may increase the concentration of blood in the eye area, such as bending over, lifting weights, crying, physical exercise, fluctuating environmental temperatures, sports, and placing foreign objects in the eyes (such as eyelashes). Also, patients should avoid driving until they are completely free of restrictions. Patients who wear contact lenses should avoid wearing them for 3 to 4 weeks and smoking and drinking alcohol are prohibited for 3 weeks after surgery.

Potential risks and complications 

Among the risks of blepharoplasty are:

  • Bleeding behind the eyeball (posterior hematoma), 
  • Inability to close the eyelids (hare’s eye)
  • Outward turning of the lower eyelid (ectropion)
  • Dry eye
  • Photosensitivity
  • Persistent pain
  • Hare’s eye

Any man or woman who wishes to treat the aesthetic problem of puffy eyes and droopy eyelids is a good candidate for blepharoplasty. Most patients who decide to undergo this procedure are over the age of 35. However, if ptosis may develop at a younger age due to heredity, you may proceed with the procedure according to the advice of the plastic surgeon, Dr. Athanasios Athanasiou.

The results of blepharoplasty are visible for 10-12 years after the procedure. The skin that is removed from the area will not appear again; however, the effects of gravity and time can not be stopped. Achieving a better look and stalling the effects of aging is possible using effective, modern and safe methods.

Edemas will remain for 5-7 days although this depends on the technique and approach used by the surgeon and your type of work. It is advisable that you wear sunglasses during recovery, you should also avoid lifting weights and engaging in intense activities.
