The world’s foremost scientific body for aesthetic plastic surgery (ISAPS) reports that it is the third most common surgical aesthetic procedure worldwide, after blepharoplasty and liposuction.

Breast augmentation methods

There is no single method that is ideal for every woman. The right choice depends on anatomy, the desired size, tissue quality and the type of result the woman wants. The three main techniques are:

Augmentation with silicone implants is the most widely used and predictable method for increasing breast volume. Modern implants have a silicone shell and contain cohesive gel, with a firmer consistency and a more natural texture and feel compared with older materials.

Implant selection is based on the diameter of the breast base, tissue thickness, skin elasticity and the result the woman wants. No single shape suits every woman; the right implant is the one that matches the anatomy and aesthetic goals of each individual case.

The plastic surgeon uses two types of implants: round implants and anatomical implants (teardrop-shaped).

Implant types

Round implants distribute volume more evenly and provide greater fullness in the upper pole of the breast. Anatomical implants have greater projection in the lower pole and follow the natural slope of the breast more closely.

Implants can be placed under the mammary gland, under the fascia of the pectoralis major muscle, or under the muscle itself.

The most suitable plane is chosen during preoperative planning, based on the shape of the chest and breast, tissue thickness and the specific measurements taken during the medical examination.

The implant is usually inserted through an inframammary incision, located in the natural fold beneath the breast, or through a periareolar incision, at the edge of the areola.

Breast augmentation with fat, or autologous fat transfer, uses adipose tissue from the woman’s own body. Fat is harvested through liposuction from areas such as the abdomen, waist, thighs or flanks, undergoes specialised processing and is then injected into the breasts. At ABH Medical Group, fat harvesting can be performed with VASER, combining breast enhancement with sculpting of the donor area.

This method is mainly indicated for women who want a mild increase without implants, shape improvement, or correction of minor asymmetries. The result has a natural look and feel, but requires an adequate amount of available fat. As part of the transferred fat is naturally reabsorbed by the body, the final volume is assessed once the result has stabilised, and in some cases a supplementary session may be needed.

Hybrid breast augmentation combines a silicone implant with autologous fat. The implant provides the primary volume and the desired projection, while the fat is selectively injected where needed, to discreetly cover the implant edges and create a smoother transition to the surrounding natural tissue.

This method is particularly useful for slim women or women with thin tissue, in whom the implant outline may be more noticeable. It can also help correct minor asymmetries and allow more detailed shaping of the breast. When the fat is harvested using modern Vaser liposuction technology, the contour of the abdomen, waist or the area from which the fat is removed is improved at the same time.

The Breast Augmentation Procedure

1. Initial Consultation

Your medical history is taken, your goals and expectations are discussed, and your anatomy, tissue quality and breast condition are assessed.
The breast base and key anatomical distances are measured, the appropriate implant, placement plane and incision site are selected, and a three-dimensional simulation of the likely result is presented

The necessary tests are carried out, and instructions are given regarding medication use or discontinuation, fasting, and having someone accompany you on the day of the procedure.

After general anaesthesia is administered, the appropriate pocket is created and the implant is placed according to the preoperative plan. The procedure usually takes 1-2 hours.
After the procedure, you remain under observation, a special support bra is fitted, and you are given instructions on medication, incision care and the first few days at home.
Return to daily activities happens gradually. The special support bra is worn for as long as advised by the doctor, while scheduled follow-up visits allow monitoring of healing and the progress of the result.

Cost of Breast Augmentation

The cost of the procedure is individualised and depends on the type, brand, shape and profile of the implants, as well as the surgical method chosen. For example, the cost differs between augmentation with implants alone and hybrid augmentation, where implants are combined with simultaneous fat transfer. What is considered the ideal breast?

The ideal breast is considered to be one that is in harmony with each woman’s body type and is based on her anthropometric characteristics (height, chest circumference, existing breast size) but also on her personal preference, always combined with the experience and aesthetic sense of the plastic surgeon she has chosen. The ideal breast follows the 45:55 ratio rule, meaning 45% of breast volume should sit above the nipple and 55% below it. This ratio allows the surgeon to avoid an overly full, artificial-looking décolletage and to create a breast with a natural slope, a gentle transition in the upper pole and better balance in the lower pole. This “gold standard” ratio is nothing new, as it can be found in classical works of antiquity. To achieve this ideal ratio, besides surgical technique, choosing the right implant is of the utmost importance.

An important part of every preoperative assessment, beyond the necessary tests, is a mammogram and/or breast ultrasound. This examination protects the patient from any undiagnosed breast conditions and also provides suitable baseline images for later comparison.

It is generally recommended to stop vitamin supplements, smoking, alcohol and aspirin 1-2 weeks before the procedure and immediately afterwards. A healthy diet contributes to a better and faster recovery.

The inframammary approach with implant placement beneath the pectoral muscle (dual plane technique) is usually preferred. Implants are placed according to the latest international surgical technique guidelines (the “14-point plan”), aimed at minimising the likelihood of complications associated with the procedure.

In cases where breast ptosis is also present, it is recommended that breast augmentation be combined with a breast lift (mastopexy).

Also, in cases where liposuction has previously been performed, it is recommended to enhance the implants with autologous fat (lipofilling) for even more natural breast augmentation results, also known as hybrid breast augmentation.

The silicone implants we use at ABH have a very long lifespan and carry all the relevant certifications (FDA, CE). However, a woman may choose to have them replaced after some years, due to ageing, pregnancy, etc. In most cases, the implants do not need to be changed.

The results of breast surgery are immediately visible. For the first few weeks, there is mild swelling, which gradually subsides as the breast takes on its final shape.

A woman can undergo breast augmentation surgery once she reaches adulthood, at 18 years of age. There is no upper age limit for breast augmentation. The International Society of Aesthetic Plastic Surgery reports that 54% of procedures are performed on women aged 18 to 34, meaning that almost half of the women undergoing the procedure are older than that.

With modern techniques, silicone implants do not prevent breastfeeding. The inframammary incision and submuscular or dual plane placement leave the mammary gland and milk ducts intact. What is recommended for a woman who has had breast augmentation is that, during both breastfeeding and pregnancy, she wear a well-fitting bra that properly supports the breasts.

The procedure takes about 1 hour, and you can go home the same afternoon or the following day. The sutures are internal and absorbable. The first dressing change takes place the day after the procedure. The result is usually immediately visible, while the feel and texture of the breast improves gradually over the first two months after the procedure.

After surgery, you will need to wear a comfortable bra without underwire (a sports bra or nursing bra) for about a month. Some swelling is initially present; it gradually subsides, and the breast reaches its final shape after 2 months.

From the very first day after breast augmentation, you can move around freely. Returning to light work is recommended after 5-7 days. You should avoid sudden movements, lifting weights and driving for 2-3 weeks. After the first three weeks, the patient can return to all daily activities without particular restrictions.

Sexual activity is also not permitted for at least one week.

The incision appears as a thin red line beneath the breast or around the areola, and its final appearance depends on skin type and proper adherence to postoperative instructions. It usually becomes inconspicuous over time.

Breast augmentation is a predictable and safe procedure when performed by a specialised plastic surgeon, in a properly organised surgical environment, and with certified implants.

Swelling, bruising, a feeling of pressure and possible changes in sensation are expected in the first few days, and usually subside gradually with appropriate treatment and care.

Less commonly, haematoma, infection or delayed healing may occur, and are managed with close monitoring by the doctor and appropriate medication.

The formation of a membrane (capsule) around each implant is an expected, normal reaction of the body. Although rare with modern implants, in some cases this capsule can harden, causing discomfort or a change in breast shape, a condition known as capsular contracture. It is usually mild and manageable, but in severe cases it may require corrective surgery.

Implant displacement, visible rippling or rupture are also possible, but occur quite rarely in medical practice and are managed on a case-by-case basis.

With the correct surgical technique, careful tissue handling and appropriate implant selection, the risk is minimal.

Contrary to various rumours circulating on social media, the U.S. Food and Drug Administration (FDA) states that no link has been found between silicone implants and breast cancer, connective tissue disease, or reproductive problems.

A permanent and substantial breast augmentation without surgery does not exist. The only alternative to implants is fat transfer.

Although hyaluronic acid is used with great success in other aesthetic treatments, its use in the breast is not medically recommended, as it can interfere with mammographic screening for breast cancer. Besides being temporary and costly, injecting a large volume of hyaluronic acid into the breast is associated with an increased risk of infection, inflammation, asymmetry and material migration.
