What is Hair Loss?
Hair loss is one of the most common dermatological problems that affects both men and women. The most common form of hair loss is seasonal hair loss.
The extent and cause of hair loss can vary from person to person and may be associated with various factors such as genetic predisposition, hormonal changes, diet, stress, medication use, as well as environmental and psychological factors.
When hair loss is observed throughout the year and especially when there are visible signs of thinning on the scalp or body, it requires evaluation and treatment by specialized doctors. To achieve effective treatment, it is necessary to address the issue of hair loss in its early stages.
Normal Hair Loss
On our head, we have approximately 100,000 to 150,000 hairs. Every day, we lose about 80 to 100 hairs, which is not a problem as hair is constantly renewing.
Seasonal Hair Loss
Seasonal hair loss is considered the most common form of hair loss and is mainly observed during seasons with temperature changes such as spring and autumn. According to research, the increase in hair loss during the autumn months is related to the increased growth of hair that occurs in July, aiming to protect from UV rays.
Seasonal hair loss is temporary, and its management is a relatively simple process. Typically, the use of appropriate shampoos and incorporating dermato-cosmetic products into the daily routine is recommended, while proper and balanced nutrition is extremely important.
Pathological Hair Loss
In pathological hair loss, the lost hairs are not replaced, resulting in a decrease in the overall hair density, known as thinning. This condition is usually referred to as “Alopecia.”
What is the Hair Growth Cycle?
Each hair is a living organism, an organ of the body that is produced from the hair follicle and goes through three stages:
- The anagen phase, during which the hair grows and lengthens. This phase lasts 2-6 years in adults, as hair grows on average about a centimeter per month.
- The catagen phase, which begins once the anagen phase is completed and lasts 2-3 weeks. During this stage, the hairs stop lengthening and prepare to fall out.
- The telogen phase, which lasts approximately 1-3 months and gradually sheds from the body.
In a person not experiencing pathological hair loss (Alopecia), 90% of the hairs are in the anagen phase, 1% in the catagen phase, and 9-10% in the telogen phase. Essentially, Alopecia occurs when there is a higher percentage of hairs in the telogen phase.
What are the main categories of Alopecia?
The types of Alopecia are divided into two major categories, scarring and non-scarring Alopecia.
Scarring Alopecia are the categories where hair loss is due to permanent destruction of the hair follicles. These categories are often associated with:
- Injuries
- Burns
- Chronic inflammatory diseases
- Microbial infections
Non-Scarring Alopecia categories in which there is no destruction of the hair follicle during hair loss, nor is there scarred tissue. These categories include:
- Androgenetic (Androgenic) Alopecia
- Circular (Trichotillomania) Alopecia
- Traumatic Alopecia
- Fungal Alopecia
- Trichotillomania
What are the main causes of hair loss?
The hair growth cycle can be affected by various factors, and each individual may experience hair loss for a period in their life.
The most common cause of hair loss, when not pathological, is stress. Stress plays a crucial role in our lives as it significantly affects us. Many cases of intense hair loss are directly related to high stress levels.
Iron and ferritin deficiency in the blood, thyroid disorders, and high levels of androgens are equally important factors leading to hair loss.
However, we should not overlook diet. Unbalanced nutrition and exhausting diets strain the body and can even affect the hair.
Additional factors contributing to hair loss include:
- Hormonal changes
- Genetics
- Age
- Autoimmune diseases
- Various medications
- Sun exposure
- Injuries
- Burns
What is Androgenetic Alopecia?
Androgenetic Alopecia is the most common form of hair loss and is found in men, in about 95% of cases, but also in women. The main reasons for the development of Androgenetic Alopecia are attributed to genetic-hereditary factors and hormonal imbalances.
Androgenetic Alopecia in Men
Heredity is the strongest factor in the manifestation of hair loss in men. Genes determine to a large extent if male pattern baldness will occur, at what age, with what pattern, and what the degree of progression will be.
Men with a hereditary predisposition usually show signs of Androgenetic Alopecia after adolescence, where rapid thinning with intense progression of thinning occurs, while in the decade of the 20s to the 30s, it appears with slower progression rates. It is important to mention that once this phase begins, there is no way to stop it. The treatments we apply aim only to delay its progression.
Androgenetic Alopecia in men is expressed by a receding hairline, temporal areas, and the crown of the head. The assessment of Androgenetic Alopecia in men is done according to the Norwood scale.
Androgenetic Alopecia in Women
Women are less likely to develop Androgenetic Alopecia. In women, thinning is observed at the parting and gradually spreads across the entire scalp. The assessment of Androgenetic Alopecia in women is done according to the Ludwig scale.
What diagnostic methods do we use for the diagnosis of Alopecia?
For the correct diagnosis of Alopecia, a series of actions by a specialized physician is required.
- Macroscopic – Clinical examination
During visual inspection we observe the expression of dilution. That is, if the thinning is located throughout the head, if it is local or diffuse and if it follows the pattern of Androgenetic Alopecia. Of course, it should be accompanied by a skin examination for the presence of fungal or microbial infections. - Medical History
Taking a thorough medical history aims to determine if the use of any medication has affected the condition of hair loss. - Family History
Taking family history aims to discover the effect of genetic factors on hair loss. - Gene Tests
In addition, there are gene tests (DNA Trichotest) that we can conduct, if necessary, to have a more complete picture of the predisposition and progression of hair loss. - Trichogram
The Trichogram is the examination during which a graft is taken from the follicle to investigate the lesions of the hair follicle. The result of the examination will determine the type of hair loss and will contribute to the effective treatment of the disease.
Treatments for Androgenetic Alopecia
In deciding the appropriate treatment, it is important to take into account the age, the stage of thinning, the lifestyle of the person and especially his expectations but also how much this condition affects him psychologically. As we have mentioned, androgenetic alopecia can not be stopped or stopped. Therefore, all treatments are applied to delay its progression or restore the damage it has caused to the scalp, such as that of thinning.
